Continuity of Carer

We have added a question to our booking questionnaire to cover this new data collection but wondered how anyone has approached the following...

  • number of women who during that month are placed onto a continuity of carer pathway after the antenatal booking appointment.

We thought about adding a question to the AN contact?

Alistair - Have Euroking developed any model questions to cover this?

    I am not sure how many of you (Like me!) had misunderstood the requirements for data collection for mothers moving onto a Continuity pathway after booking in March. As below this is a one off data submission.

    NHS England have advised:

    As we discussed the March 19 measure is a one off exercise and we will then be moving to BAU measurement through MSDSv2. We are confident nationally that data fields currently set out in the broad data specification of MSDSv2 can be used to help us track receipt of continuity at team and provider level. The missing piece of the puzzle here is which of these data fields trust information systems are able to submit routinely to MSDSv2. I understand NHS Digital have recently received returns from all trusts on this as part of their state of readiness exercise. As soon as we have the findings of this we can set out to providers what needs to be done to measure CoC consistently at a national level to satisfy the national criteria that we are agreeing for the purposes of measurement. In the meantime, the Measurement and Evaluation Framework published by the RCM sets out that we will be looking at whether a woman has been placed on a CoC pathway and has a named lead midwife and team in her personal care plan, and then judging against that whether it was this named lead midwife or team involved in her antenatal, intrapartum and postnatal midwife appointments. I hope that’s helpful in the meantime?

     

    To clarify for Euroking users Alistair has additionally advised:

    Attached is the specification for MSD102 – a table within MSDS v2 which contains items regarding Careplans and the provision of COC. I’ve highlighted where the data item being collected from 1st March sits within this table and the Individual / Team identifiers that would be allocated as per the provision of care relating to the occurrence of the plan.

    MSD102 Maternity Care Plan Prev MAT301

    Description

    To carry details of the Care Plan during the current Maternity Episode.

    Multiple occurrences of this table are permitted.

    This data item indicates where a care plan is in place, including personalised care plans, in order to meet the requirements of the Better Births report.

    The data items in this table relate to antenatal, birth and postpartum care plans, and captures the initial plan and subsequent changes to each care plan.

    For the purpose of this data set, antenatal and postpartum care plans only capture the type of lead care professional, and changes in type of lead care professional. The birth plan captures additional information, specifically related to place of birth.

    The first antenatal plan should be produced at the booking appointment and reviewed at subsequent antenatal appointments. Any changes thereafter are largely dependent on the outcome of screening tests and antenatal complications.

    Changes to birth plans will be captured to the point of ONSET OF ESTABLISHED LABOUR DATE TIME. Any changes thereafter will be classed as transfers and indicated by differences in planned and actual place of delivery.

    What this effectively means is that every time a Careplan is created and revised, a separate instance of MSD102 is returned. So for example of a plan was created at booking – that’s one instance of MSD102. If then, at an antenatal appointment the plan is revised, that would cause a second iteration of MSD102 to be created and so forth.

     

    In terms of COC specifically, once a Care Professional Local Identifier and Team Identifier has been established, this is then used as follows:

     

    The Care Professional Local Identifier is captured in several tables

    • MSD102 Maternity Care Plan,
    • MSD202 Care Activity (Pregnancy),
    • MSD302 Care Activity (Labour and Delivery),
    • MSD401 Baby's Demographics and Birth Details (N.B. named 'Care Professional Local Identifier (Delivering Baby)'),
    • MSD405 Care Activity (Baby) and
    • MSD504 Assigned Care Professional) to allow the recording and subsequent monitoring of continuity of carer information, where applicable. The Care Professional Team Local Identifier is captured in several tables

    • MSD102 Maternity Care Plan,
    • MSD202 Care Activity (Pregnancy),
    • MSD302 Care Activity (Labour and Delivery),
    • MSD405 Care Activity (Baby) and
    • MSD504 Assigned Care Professional) to allow the recording and subsequent monitoring of continuity of carer information, where applicable The data collected for establishing if the woman is on a Continuity Pathway at booking is being collected alongside a named professional and allocated to a team – which would serve the demand of the Care Professional Local Identifier and Care Professional Team Local Identifier above. As of today, this is currently only established at booking within EuroKing however we are looking to extend this with revision capabilities in subsequent contact encounters so the full COC pathway can be tracked both within EuroKing and represented within MSDS 2.
    • Therefore we currently do not require to add Continuity questions with the AN and pre delivery workflows .

    we have requested the following:

    Continuity questions to be added to all AN workflows & Pre delivery , to be positioned after Care changed question. Answers to pull through and be visible if already answered, but allow updating

    Q1 Was the mother moved onto a continuity of carer pathway after booking

    Q2 Which continuity team was the mother placed with after booking within following workflows:

    1. AN contact
    2. Delivery suite Triage
    3. AN Discharge
    4. Pre delivery – insert after AN care received

    I have also contacted NHS England to discuss this for clarity & to ensure consistency of data collection across trusts and Maternity system suppliers – I will update you when I hear from them
