I have emailed this to the user group as well as posting on here so apologies if you read this twice.
We are looking at changing how we record our PN care. Once discharged home, if a patient needs to be seen for PN care in hospital they are currently seen on delivery suite. In the past we have always recorded these on Euroking using the PN discharge questionnaire. That meant that all PN care was recorded in the same questionnaire whether the patient was seen for a few hours or stayed for a few days.
Going forward, we are hoping to now record contacts and admissions separately. So, for those women who are seen and discharged a PN contact questionnaire would be completed. For those who are seen and then admitted then a PN contact is completed initially followed by the PN discharge questionnaire.
How do you record similar events at your unit?
How do you differentiate between what is classed as a ‘contact’ and what is classed as an ‘admission’?
Do you classify based on length of stay?
